PARTNERSHIP APPLICATION DOCUMENTATION

Application Documentation List

Thank you for your interest in becoming a homeowner through Lubbock Habitat for Humanity (LHFH). In order to complete the application, we will need the following information.  Please bring or mail copies; or bring originals and we will make copies.

  1. ___ $25 application fee ($25 per applicant) for background and credit report – turn in with the application titled – Application-Habitat Homeownership Program

DO NOT TURN IN THE INFORMATION LISTED BELOW UNTIL REQUESTED

  1. ___ Workshop Certificate – Good for one year from the date of the workshop
  2. ___ Photo ID
  3. ___ Social Security Card
  4. ___ Permanent Residence card or INS Certification of Citizenship (if applicable)
  5. ___ Landlord Verificationsee form (Please note if you are receiving any type housing assistance)
    1. ___ Former landlord verification (if less than 3 years at current address) – see form
  6. ___ Rent receipts (last 6 months, consecutive, receipts)
  7. ___ Employment verification (stamped, faxed or mailed by employer) – see form
    1. Former employment verification (if less than 3 years in current job) – see form
  8. ___ Paycheck stubs (6 months) – 6 months must be consecutive
  9. ___ Other Income: Award letters from any that apply
    1. ___ SSI disability
    2. ___ Child support records/court order
    3. ___ Social Security pension
    4. ___ Gift letter
    5. ___ 401 (k) retirement
    6. ___   Alimony
    7. ___ VA
  10. ___ Assistance
    1. ___ Section 8 Housing
    2. ___ Food Stamp Certification
  11. ___ Bank verification (if you have a savings and/or checking account) – see form (Include a form for the checking and one for the savings accounts)
    1. ___ 6 months of consecutive checking account statements
    2. ___ 6 months of consecutive savings account statements
  12. ___ Tax returns and W2 forms (most recent – 2 years)
  13. ___ Divorce decree (if applicable)
  14. ___ All recent credit card bills
  15. ___ Recent copies of other bills and loans such as but not limited to: 
    1. ___ car payment
    2. ___ furniture
    3. ___ cell phone
    4. ___ rent-to-own
    5. ___ Pay day or title loans
    6. ___ medical bills will be reviewed to determine debt ratio
    7. ___ student loans
    8. ___ utility bills
    9. ___car insurance
    10. ___ medical insurance
    11. ___ other; – LHFH will pull credit report after you turn in your completed application. If the credit report shows that you are not paying on your student loans, LHFH will require you to call the appropriate agency to get the information showing when you will begin making payments and the monthly payment amount”.
